Patient Advocate & Professional Caregiver (Davis)

Hi, I’m Stan and have over 10 years of experience providing excellent caregiving services. I’ve worked with patients/clients in hospitals, assisted living facilities, nursing homes and private/family homes. I’ve provided care and support services for adults of all ages with a variety of injuries, illnesses, conditions and disabilities including physical, mental, emotional and developmental. Advanced services that enhance my caregiving consist of the following patient advocate services:  Supporting and working with clients who are patients in hospitals or nursing homes as well as clients residing in assisted living facilities or in their own private/family home  Protecting the rights of clients by ensuring that they are aware and understand their rights and recognize their freedom of choice  Helping empower clients with information they need to make sound decisions  Providing clients with person-centered care and support while prioritizing their safety and well-being  Documenting clients’ complaints/issues and taking necessary steps to help resolve them  Ensuring that clients have a voice, their concerns are heard and properly responded to  Facilitating communication between clients and third parties/medical and residential staff as deemed appropriate by clients  Providing moral and emotional support for clients  Facilitating communication between clients and their family by assisting with phone calls, Facetime, etc.  Providing family with reports and updates as deemed appropriate by clients  Developing/managing individually customized service care plans for clients to receive ongoing or short-term supported living services (SLS) including caregiving support Standard/basic caregiving services that I offer consist of the following:  Providing personal health and hygiene care that include recording/monitoring vitals; administering shots/injections; performing catheter care; implementing seizure management protocol and assisting with bathing, dressing, grooming and oral care  Assisting with medication/treatment regimen including physical and occupational therapy  Providing mobility support and safely performing transfers  Performing light housekeeping tasks including laundry  Assisting with shopping, errands and appointments  Preparing meals and assisting with feeding  Providing companionship as well as facilitating socialization I am certified through Home Care Aide (HCA) and In-home Support Services (IHSS). I am also nurse-delegated as well as CPR/First Aid certified and can provide references upon request.
